Webinar Wednesday - Information sharing and continuity of care for marginalized patient populations

One of the priorities for health systems across Canada has been improving access and providing quality care for marginalized patient populations and clients most impacted by the social determinants of health. However, caring for this population can be challenging as they often do not have a stable medical home, and coordinating care across multiple organizations and sharing data freely presents logistical, technical, and privacy legislation hurdles. 

In this webinar, Inner City Health Associates (ICHA) discusses their transformative approach to population health through street outreach initiatives, transitional primary care, and collaboration with hospitals and community health organizations. Discover how ICHA’s team of highly mobile healthcare professionals leverages technology to enable collaborative care amongst their team of providers for the marginalized patient populations they serve. Key learnings will be shared from the development of their programs, including how to enable communication amongst providers, information sharing between organizations and approaches to coordination of care across the continuum.

Presented by: 

Alena Ravestein is a Senior Manager at Inner City Health Associates responsible for systems design and program development.  She co-chairs both the Digital and Privacy working groups for the Downtown East Toronto OHT and contributes to other regional initiatives like the Homelessness Health Services Framework and Privacy Community of Practice.  Alena is passionate about system interoperability, patient-centred data sharing and creative improvements to the way we collectively deliver care.

Shaye Martorino has been a Registered Practical Nurse for a decade after graduating from Sir Sanford Fleming in Peterborough, Ontario. Throughout her career, Shaye has accumulated diverse clinical experience, spanning various healthcare settings, including hospitals and community environments. Since the onset of the pandemic, she has worked for Inner City Health Associates (ICHA) caring for people experiencing homelessness who are impacted by COVID-19, initially as a staff nurse and charge nurse. Since February 2021, she has held the position of Clinical Nurse Manager in which she is responsible for the operational, clinical, and strategic oversight of the ICHA nursing services in several shelters across Toronto. Shaye has a passion for harm reduction and caring for people who are experiencing homelessness.
