This webinar will outline the virtual care story at Hamilton Health Sciences as the organization shifts to long-term sustainment of virtual care. This journey has three phases: 1 - Past (pre-pandemic); 2 - Present (pandemic) and 3 - Future (two years later). 

The current phase includes collaborating with a new electronic medical record implementation team to set up integrated virtual visits, establishing new workflows, and creating organizational resources, such as playbooks, to tie it all together. The team will share experiences forming partnerships with peer hospitals via a virtual care community of practice to share learnings. 

Presented by: 

Clare O'Connor, Manager, Virtual Care has been a nurse for 16 years, graduating from McMaster University and started her career in the Emergency Department at Hamilton Health Sciences. She went on to pursue a non-clinical opportunity as a patient safety specialist, which got her interested in quality improvement and led her to obtain a masters in Quality and Patient Safety at U of T in 2013. She then took a role as a continuous quality improvement coach, helping to spread and sustain the lean management system for quality improvement at HHS. She is also a steering committee member of the McMaster Department of Medicine’s Quality Improvement community of Practice. Since the fall of 2021 she has worked as the interim manager of virtual care at HHS as the organization continues to scale their VC offerings and move into a new EMR system. Outside of work she stays busy with her husband and their three small kids, and enjoys yoga, spin, and baking.

Pooja Patel, Specialist, Virtual Care with Hamilton Health Sciences currently implements and manages virtual care projects at Hamilton Health Sciences with clinical and corporate teams. Pooja previously worked with the B.C. Provincial Health Services Authority implementing virtual care solutions, such as virtual visits, text messaging platforms, and mental health tools with multidisciplinary clinical teams and patients. She holds a Bachelor of Health Sciences from Western University and a Master of Health Administration from the University of British Columbia. She is also the recipient of the 2020 Emerging Leader Award from Digital Health Canada and recently obtained her PMP designation. Pooja enjoys exploring the outdoors, writing,  and seeking solutions to the climate crisis.

Julie Matthews, Specialist, Virtual Care with Hamilton Health Sciences, the only health system in Ontario that cares for all ages, from pre-birth to end-of-life. Prior to her experience on the Virtual Care Operations Team, Julie held several roles in Organizational Development, supporting large-scale changes across the organization. Julie holds a Bachelor’s in Business Administration (BBA) from Wilfrid Laurier University and is currently completing a Master’s in Public Administration at Western University.
