Webinar Wednesday - Virtual Pulmonary Rehabilitation: A New Reality

This presentation will highlight chronic obstructive pulmonary disease (COPD) and the St. Joseph's Healthcare Hamilton experience in implementing virtual pulmonary rehabilitation in the Respiratory Rehabilitation Day Program and the Post-Discharge COPD Clinic to improve health outcomes for patients. This project included a hospital information system integration with the vendor to support interoperability, documentation, adoption, and clinician efficiency. The technical specifications will be outlined and the lessons learned and evaluation components will be highlighted for providers seeking to implement virtual pulmonary rehabilitation within their practice.

Presented by: 

Dr. Natya Raghavan, MDCM, FRCPC, has been on staff at McMaster University since 2012 working at both Hamilton General Hospital and St Joseph’s Healthcare. She provides clinical care in Respirology and Internal Medicine at both sites.  She has been the Physician Lead of the Respiratory Rehabilitation Program since 2019 and has navigated the changing landscape of out patient medicine in frail patients with lung disease during the pandemic which has forced a shift to virtual care.  She is also the lead of the Virtual Care Task Force at Hamilton Health Sciences.

Carina Andreatta is currently the Program Manager of the Project Management Office (PMO) Analytics departments and Telemedicine Office at St. Joseph’s Healthcare Hamilton. She has a passion for creating meaningful and valuable ways to allow patients to take better control over their health using digital tools and technology. Carina graduated from Western University with an Honors Specialization in Health Sciences and a Minor in Psychology. She pursued her interest in technology and patient-centered care through her post-graduate education at McMaster University where she completed a Master’s of Science, in eHealth. She has worked at SJHH for five years, leading Clinical Transformation, and at Digital Health Canada previously supporting various national communities of interest and working groups.

Ceara Holditch is a Project Manager at St. Joseph’s Healthcare Hamilton and at the Greater Hamilton Health Network. Her portfolio includes coordinating various digital health funding opportunities and leading the implementation of several remote patient monitoring projects. She holds an Honours Bachelor of Arts and a Graduate Certificate in Project Management.
